Purpose. To analyze a clinical case of Vogt–Koyanagi–Harada syndrome and evaluate the effectiveness of prescribed systemic corticosteroid therapy. Material and methods. Patient K., 17 years old, female, Korean, addressed the Kaluga branch of the Eye Microsurgery MNTK in early January 2023. Complaints: low vision, feeling of blurring in both eyes. According to the patient, at the beginning of December 2023, she noted redness in both eyes, and therefore contacted an ophthalmologist at her place of residence. The ophthalmologist diagnosed “Conjunctivitis OU” and prescribed the drugs Signicef and Visalergol. During the therapy, the state of visual functions worsened, and the above-mentioned complaints appeared. Results. At the time of addressing to the Kaluga branch of Eye Microsurgery, the visual acuity of both eyes was 0.3 and could not be corrected. Intraocular pressure indicators corresponded to the norm – 16 mmHg OU. Based on the complaints, medical history, ethnicity of the patient, objective examination data and the results of additional research methods, a diagnosis of Panuveitis, Vogt–Koyanagi–Harada Syndrome OU was established. Due to the fact that the patient did not have a full range of symptoms (vitiligo, graying of hair, poliosis, alopecia), to clarify the diagnosis, the patient was referred for consultation to the Federal State Budgetary Institution “National Medical Research Center for Eye Diseases named after. Helmholtz” of the Ministry of Health of Russia. At the Helmholtz Clinic, the diagnosis was confirmed and treatment was prescribed: glucocorticosteroid therapy in the pulse therapy mode (prednisolone at a dosage of 40 mg). Positive dynamics were noted at the follow-up examination in 10 days after the prescribed treatment. Conclusion. The analysis of the presented clinical case is of interest due to the rare occurrence of Vogt–Koyanagi–Harada syndrome, and most importantly, early detection – a month after the onset of the disease (which is extremely rare), when there are no extensive manifestations of the skin. Adequate therapy for this disease made it possible to achieve stabilization of the condition in a short time and avoid such severe complications as cataracts, secondary glaucoma, choroidal neovascular membrane, exudative retinal detachment, and subatrophy of the eyeball.
