Decision considerations in EOH are accommodated within the GRADE EtD framework.• Limited uptake of the GRADE EtD in EOH is likely due to other factors and not incompatibility of the EtD framework with EOH decision-making considerations.• Guidance for users is essential to improve the framework's applicability to EOH. What this adds to what was known• There are currently six GRADE EtD frameworks for different types of decisions: individual-and population-level clinical recommendations, health system and public health recommendations, coverage decisions, individual-and population-level diagnostic test recommendations.• This project introduces a seventh framework that is intended for application in making environmental and occupational health (EOH) decisions. What are the implications and what should change?• Policymakers, regulators, and other decision-makers should consider adopting the GRADE EtD for EOH to improve consistency and transparency in their approach to formulating recommendations or regulations.
