HIV is associated with low bone density and an increase in fractures through direct viral effects, medication side effects and concomitant risk factors including poor nutritional status, suboptimal calcium and vitamin D intake, hypogonadism, low body weight, and alcohol and tobacco use. Certain medications used for HIV treatment such as Neocleotide reverse transcriptase inhibitor and Protease inhibitor are associated with higher rates of bone loss than others. We present an unusual case of spontaneous hip fracture in a young HIV infected male with type 1 diabetes that was not associated with any trauma.
