Background We present a clinical case of peristomal eventration of ileal conduit repaired with a sugarbaker technique. Methods A 56 years-old man, with overweight (BMI 30 kg/m2), hypertension and surgical history of radical cystectomy and Bricker reconstruction due to urothelial carcinoma. During oncologic controls, he was diagnosed of a infraumbilical and peristomal eventration. The patient had discomfort and was referred from the urology clinic, Elective surgery was programmed after the CT scan confirmed the eventration suspicion of occupation by the colon. Results A peristomal eventration with the cecum and distal ileum inside. The content was carefully reduced and dissected, paying special attention to the mesenteric side to avoid ureteral lesions and devascularization. The size of the eventration was adjusted by using 0 barbed suture. Afterwards, an intraperitoneal mesh was placed around the stoma fixed with tackers. The post-operative period occurred without complications, and on the 2nd day, he was discharged from the hospital. Conclusions The repair of patients with ileal conduits’ eventrations can be a true challenge due to altered anatomy and the possibility of injuring the ureters, so the laparoscopic approach offers a great advantage for evaluation and repair of these unusual cases.
