A nine month old child presented with a history of recurrent respiratory distress and infections and repeated admissions to the hospital. The x-ray of the child demonstrated fluid-filled bowel loops in the thorax. Computed tomography of the chest demonstrated. bowel loops in the anterior mediastinum anterior to the heart. A diagnosis of Morgagni hernia was made. The child was operated via Robot Assisted Repair of the defect. Reduction of contents, excision of the sac and a simple closure of the diaphragm with the anterior abdominal wall was done. A transcutaneous suture was passed from the abdominal skin and through all the abdominal wall layers to anchor the diaphragm to the wall to ensure a secure closure. The suture was buried in the skin. The child recovered uneventfully and is well on follow-up after a year.
