Accessory mitral valve tissue is a rare congenital anomaly usually detected in childhood. Adult patients with accessory mitral valve tissue may present with symptoms such as dyspnoea, chest pain, arrhythmias or embolic events. Diagnosis is typically made through echocardiography, and treatment options vary depending on the severity of symptoms and the degree of obstruction. We report a case of accessory mitral valve tissue presenting as a large mass in the left ventricular outflow tract in an adult, which posed a dilemma in diagnosis.
