Background: Cardiorenal syndrome is a disorders involving both the heart and kidneys in which acute or chronic dysfunction in one organ may induce acute or chronic dysfunction in the other organ. This case report show how cardiac disfunction leading to cardiorenal syndrome. Case Illustration: A 64-year-old woman complained for shortness of breath and chest pain a week before arrived at ED. Patient’s vital sign BP 180/100, HR 101, SpO2:92% room air, has a history of diabetes, and hypertension on regular medication. ECG revealed ST depression in I, aVL, V3-V5. Chest X-ray shows Pulmonary Oedema. Echocardiograph resulted in RWMA (+), ejection fraction 46%. Laboratory result in high creatinine concentration leading to renal dysfunction with eGFR 21 mL/min/1.73m 2 (CKD-EPI 2021), and high troponin I level 6.07. The patient was given ISDN and furosemide injection for her hypertensive emergency and pulmonary oedema therapy also heparin injection as ACS treatment with renal dysfunction. Discussion: cardiorenal syndrome type 2 is described by chronic cardiac abnormalities resulting in impaired renal function. Result: after 3 days of treatment for acute lung oedema, complaints of shortness of breath had improved, the patient had kidney function re-checked and there was improvement to eGFR 48 mL/min/1.73m 2 . Patient then underwent a PCI after Coronary angiography resulted stenosis 80-90% proximal-mid RCA, 90% stenosis ostial PDA. Conclusion: An adequate treatment of cardiac dysfunction in cardiorenal syndrome can also improve renal condition.
