Macrophages are a kind of immune cells derived from monocytes in human body. They participate in and regulate the specific or non-specific immunity of human body. Different components of microenvironment stimulus will make them polarize into different types and express different physiological and pathological effects. Previous studies have shown that the amount of macrophages and the proportion of their types vary in the normal physiology and pathology of the ovary, suggesting a strong link between macrophages and ovarian function. In this review, we focused on the mechanism of action and research progress between ovarian macrophages and normal ovarian cycle, premature ovarian failure, polycystic ovary syndrome, and ovarian cancer, in order to find the key targets of ovarian macrophages in different ovarian diseases, and then conduct in-depth studies on key parts and links to better treat female ovarian and even gynecological diseases.
