Idiopathic Post-Colectomy Pan-Enteritis (PCE) is a rare, often misdiagnosed and potentially life-threatening complication of colectomy for Ulcerative Colitis (UC), characterized by an early-onset diffuse, ulcerative mucosal inflammation, possibly involving the entire small bowel. Therapy is empirically based on corticosteroids and use of anti-TNF, azathioprine and cyclosporine has been reported for steroid-refractory cases. The need for revisional surgery is uncommon.
