However, most of these studies did not include patients who have received neoadjuvant chemotherapy (NAC). The purpose of this study was to verify relationship between overall survival (OS) and number of DLNs in patients with radical cystectomy (RC) following NAC for muscle-invasive bladder cancer (MIBC). METHODS: This retrospective study considered patients who were diagnosed with clinical ≧T2N0M0 MIBC and treated with RC at Nagoya University Hospital and affiliated hospitals from January 2004 to August 2020. The association between prognosis and number of DLNs was investigated. RESULTS: We retrospectively enrolled a total of 507 patients. The mean number of DLNs was 14. 243 patients (48%) received NAC. ≧15 DLNs correlated with better 5-year OS across all patients (68.2% vs. 56.2%, p<0.01). In patients who received NAC, there was no difference in overall survival according to the number of DLNs (66.7% vs. 69.4%, p=0.777). In patients who did not received NAC ≧15 DLNs was associated with significantly better OS (70.1% vs. 46.9%, p<0.01). A greater number of DLNs was not associated with better OS in NAC patients (HR 1.27, 95% CI 0.78-2.05, p=0.33) under multivariate analysis. CONCLUSIONS: No association between a large number of DLNs and prognosis was found in patients who underwent NAC. Conversely, a large number of DLNs is desirable for patients who have not received NAC.
