METHODS: In this retrospective study, we compared recurrences within three years of nephrectomy versus recurrences after three years patients. The logistic regression analysis was performed to evaluate risk factors for late recurrence. Kaplan-Meier curves were utilized to evaluate survival, and cox-regression analysis was performed to figure out the risk factor hazard ratio compared to the non-recurrence group. RESULTS: The late recurrence group had a considerably younger baseline age than the early recurrence group. Late recurrence group had lower pT stage, low portion of papillary type, and lower incidence of sarcomatoid differentiation. However, multiple logistic regression analysis showed only the absence of sarcomatoid differentiation as the risk factor. The Kaplan-Meier analysis showed that the late recurrence group had considerably higher rates of overall survival and cancer-specific survival. A cox-regression analysis comparing overall survival between the non-recurrence group and the late-recurrence group indicated higher preoperative Crp, higher pT stage, the absence of papillary type, the presence of necrosis, and the presence of capsule invasion as significant risk factors. CONCLUSIONS: Patients with greater preoperative C-reactive protein levels, higher pT stage, and evidence of necrosis or capsule invasion should be continuously followed because of the increased risk of late recurrence. Patients with papillary type or sarcomatoid differentiation are more likely to recur earlier and should be followed often for the first three years after nephrectomy.
