METHODS: 78 RP patients with ISUP grade group 1 PCa at biopsy who underwent staging PSMA PET/CT were identified. Pathologic characteristics during RP according to PSMA findings were assessed. Adverse pathology was defined as ISUP grade group 3 at RP or non-organ confined disease. RESULTS: Of the 78 patients, only 11 (14.1%) had ISUP 1 at final pathology. 48 (61.5%), 14 (17.9%) and 3 (3.8%) had an upgrading to ISUP 2, ISUP 3 and ISUP 4-5, respectively. Median SUVmax in the prostate was 7 at PSMA PET/CT. Median PSA was 11.1 ng/ml. Most (67.9%) patients had organ confined disease at final pathology. Patients with vs. without adverse pathology (n=30) had a significantly higher median SUVmax (14.3 vs 5.3, IQR: 6.2-23.9 vs. 3.4-9, p<0.01). No difference in median PSA between patients with and without adverse pathology was recorded (p=0.2). CONCLUSIONS: Intraprostatic PSMA uptake was found to be associated with adverse pathology at the time of radical prostatectomy in patients with biopsy ISUP grade 1.
