To assess the perioperative tumor characteristics of patients staged with PSMA PET/CT prior radical prostatectomy (RP). METHODS: Within a high-volume center database, we identified patients who had a PSMA PET/CT prior to RP between 2016 and 2021. Tumor characteristics of miN0M0 vs. miM1 (+/- miN1) and miN1 patients were assessed. RESULTS: 992 patients who had a PSMA PET/CT prior to RP were identified. Of those, 802 (80.8%) vs. 112 (11.3%) vs. 78 (7.9%) had miN0M0 vs. miN1 vs. miM1 disease. miM1 patients more frequently harbored D'Amico high risk criteria (71.8% and vs. 43.6%, p<0.001), pathologic stage ≥T3b (61.5% vs. 21.8%, p<0.001) and pathologic ISUP grade 4-5 in the prostatectomy specimen (47.4% vs. 13.3%, p<0.001) compared to miN0M0 patients. In patients without neoadjuvant hormone therapy no difference in median PSA prior RP (8.9 ng/ml vs. 9.0 ng/ml, p=0.6) between miM1 and miN0M0 patients were recorded. Similarly, miN1 patients more frequently harbored D'Amico high risk criteria (67.0% vs. 43.6%, p<0.001), pathologic stage ≥T3b (66.1% vs. 21.8%, p<0.001) and pathologic ISUP grade 4-5 in the prostatectomy specimen (44.6% vs. 13.3%, p<0.001) compared to miN0M0 patients. In patients without neoadjuvant hormone therapy miN1 patients had higher median PSA prior RP (12.5 ng/ml vs. 9 ng/ml, p=0.02) compared to miN0M0 patients. CONCLUSIONS: miM1 and miN1 PCa patients in PSMA PET/CT harbor more unfavorable pathologic characteristics compared to miM0miN0 patients during RP. Only small differences in PSA prior to RP were recorded.
