We want to investigate associations between normalized apparent diffusion coefficient (NADC) value and clinicopathological characteristics of bladder cancer, and to determine whether the inclusion of NADC value can improve the performance of VI-RADS in diagnosing MIBC. METHODS: A total of 275 bladder cancer patients with preoperative mp-MRI images were retrospectively enrolled. NADC value was defined as the mean ADC value of tumor divided by that of the iliopsoas muscles in the trans caput femoris plane. VI-RADS >3 is considered MIBC. Associations between NADC value and clinicopathological characteristics were evaluated. We established models by using logistic regression analysis: VA model (VI-RADS and NADC), Images model (including variables obtained from imaging), LA model (Images model except NADC), and Full model (all significant variables). Various metrics were used to evaluate the models, including the area under the curve, sensitivity and specificity. RESULTS: NADC value was significantly lower in lesions with diameter ≥3 cm (p=0.002), MIBC (p<0.001), histological high grade (p<0.001), lymph node metastasis (p=0.006), and lymphovascular invasion (p=0.002). Compared with VI-RADS, the AUC for VA model, Images model and Full model were higher (VA model vs VI-RADS, 0.910 vs 0.870, p=0.047; Images model vs VI-RADS, 0.926 vs 0.870, p=0.004; Full model vs VI-RADS, 0.933 vs 0.870, p<0.01). No significant differences were observed among VA model, Images model and Full model (p>0.05). CONCLUSIONS: NADC value potentially reflect information about aggressiveness of bladder cancer. VI-RADS in combination with NADC value can improve the diagnostic performance of MIBC.
