Our aim was to clarify current treatment patterns and outcomes of patients with sex cord stromal tumors in Japan, and construct a web-based pathology atlas aiming to improve diagnostic accuracy and education. METHODS: This retrospective multi-institutional study was approved by the institutional review board. We solicited participation in this study from Japan Clinical Oncology Group-participating facilities and their affiliated hospitals, and facilities with previous case reports on Pubmed or Ichushi-Web (database of Japanese biomedical literature). Clinical courses and pathological results were collected. If available, two representative pathology sections (10 unstained slides per section) were voluntarily provided for central review and construction of the pathology atlas. Central pathological diagnosis was prioritized if different from the original report. Kaplan-Meier curves were created to assess recurrence-free (RFS) and cancer-specific (CSS) survival. RESULTS: Data on total of 109 patients were collected from 64 hospitals, and pathology sections were available for 89 patients. Totals of 51 patients had Leydig cell tumors, 35 had Sertoli cell tumors, 5 had large cell calcifying Sertoli cell tumors, 3 had Sertoli-Leydig cell tumors, 3 had Inflammatory and nested testicular sex cord tumors, and 12 had others. The median age at diagnosis was 42 years (range: 0-85). Twelve patients initially had metastatic disease [lung=6, retroperitoneal lymph nodes (RPLNs)=6, bone=2, brain=1, other LNs=2], 10 of them underwent systemic chemotherapy with a wide range of regimens, and 5-year CSS was 23.8% (Figure 1a). Of the 87 patients with follow-up data, 11 developed disease recurrence (RPLNs=7, lung=5, liver=1, other sites=2), and 5-year RFS was 86.3% (Figure 1b). Of the 11 patients, 9 underwent surgery for metastatic disease, and 5-year CSS was 90.9% (Figure 1c). CONCLUSIONS: The patterns of metastasis and recurrence were similar to testicular germ cell tumors. The 5-year CSS was extremely poor at 23.8% in patients with metastasis at the initial diagnosis. For recurrent cases, surgery for metastasis may be effective. We are now preparing a web-based pathology atlas.
