We extend the graft from the anterior tonsillar pillar of one side to the upper lip and the anterior tonsillar pilar of the other side in the form of omega shape METHODS: prospective study of 14 patients with pan anterior urethral strictures in whom we harvested the mega graft were included in this study. We assess donor site morbidities during the 1st week post operative for early complications and at 1 and 3 month post surgical for late donor site complications. RESULTS: The Mean harvested graft length was 16 cm (12 -21), width 1.3 cm (1.2 -1.9). The Mean harvesting time was 29 mins (25-42): The early postoperative donor site comorbidities: pain presented in all patients with median duration of pain relieving was 4 days . Late graft site complications: 4 patients had slight oral pain and numbness at 1 month and disappear totally at 3 months, 2 patients complained of reduce oral cavity at 3 month but can tolerate it, 1 patient had slight eversion of upper lip vermilion. All patients (n=14) recommended this technique to other patients in the questionnaire. CONCLUSIONS: The single extralong oral mucosal graft is a safe and feasible procedure without significant donor site comorbidities.
