Herein, we aim to assess the real-world functional outcomes of the Rezūm procedure in patients aged 75 and older. METHODS: We performed a comprehensive analysis of data from the Canadian Rezūm Registry, which includes two high-volume centers. The dataset comprises patients who underwent Rezūm treatment from 2019 to 2023 and were categorized into two age cohorts: below 75 years and 75 years or older. International Prostate Symptom Score (IPSS), Quality of Life (QoL), and maximum urinary flow rate (Qmax) were evaluated at baseline, 3- and 6-month follow-up. Descriptive statistics were analyzed using t-tests, Fisher's exact tests, or Mann-Whitney U tests. RESULTS: Out of 480 patients analyzed, data for 60 men aged over 75 and 210 men aged below 75 were available for up to a 6-month follow-up. Older patients exhibited significantly larger prostates (81.6 g vs. 71.5 g, p=0.01), lower baseline mean IPSS scores (20.6 vs. 22.4, p=0.03), and comparable prevalence of median lobes (63.6% vs. 67.4%, p=0.48). The proportion of patients achieving Minimal Clinically Important Difference (MCID) at 3 months (73.0% vs. 81.9%, p=0.152) and 6 months (82.7% vs. 87.6%, p=0.363) did not differ significantly between the two age groups. Functional outcomes between 75-year-old patients and younger patients were not significantly different at 3 and 6-month follow-up (Table 1). Furthermore, no significant change in IPSS, QoL, or Qmax from baseline were observed at any follow-up intervals between the age cohorts. CONCLUSIONS: Rezūm is a viable and safe treatment alternative for well-selected patients aged 75 and older. Our findings indicate no substantial differences in IPSS and QoL outcomes between elderly and younger patients at 6-month follow-up.
