METHODS: All patients with RCC undergoing nephrectomy in 3 regional hospitals in Hong Kong between 1992-2022 were included, excluding those with prior myocardial infarction (MI)/stroke/heart failure (HF). All patients were followed up for the outcome of major adverse cardiovascular events (MACE; a composite of MI, stroke, heart failure, and cardiovascular mortality) until 31/8/2023 or death, whichever occurred first, for a maximum of 5 years. A model predicting 5-year MACE was constructed with electronic health records-based data using Cox proportional hazards model, the discrimination and prognostic value of which were internally assessed. RESULTS: In total, 1044 patients were analyzed (mean age 60.3±12.0 years; median follow-up 5.0 years [interquartile range 2.8-5.0 years]). MACE occurred in 79 patients (7.6%). Age at nephrectomy (adjusted hazard ratio [aHR] 1.06 [95% confidence interval: 1.03-1.08], p<0.001), partial nephrectomy (aHR 0.70 [0.42-1.18], p=0.184), baseline diabetes mellitus (aHR 1.58 [0.97-2.55], p=0.064), hypertension (aHR 1.94 [1.09-3.46], p=0.025) and baseline chronic kidney disease (aHR 1.56 [0.84-2.89], p=0.159) were selected to be part of the predictive model. Within the derivation cohort, this model showed satisfactory discrimination (Harrel's c-statistic 0.755). Patients in the highest quartile of predicted risk had a significantly higher observed 5-year risk of MACE than those in the lowest quartile (log-rank p<0.001). CONCLUSIONS: A predictive model for incident MACE was constructed for Asian patients with RCC undergoing nephrectomy, achieving satisfactory discrimination and prognostic value. Further research and external validation of this model are required.
