Hunner lesion phenotype is treated with fulguration. Presently there is an incomplete understanding of the duration of the response to fulguration as well as the natural progression of the disease. We aim to characterize the time course of repeated fulgurations. METHODS: A retrospective chart review on all patients who underwent a fulguration for IC/PBS with Hunner lesions between 2015 and 2023 within an academic health system was conducted. Time to event analysis was used to assess the number of fulgurations per patient, time between fulgurations, and progression to cystectomy. Patient's time at risk was defined from first fulguration to cystectomy or last known follow up. RESULTS: 94 total procedures were performed on 40 patients. The mean age of the cohort was 69.3 years, and 87.5% were women. Mean follow up time was 30 months. 27/40 (67.5%) received 1 repeat fulguration, 16/40 (40%) received 2, and 6/40 (15%) received 3+. At 12 months, 45% of patients required a second fulguration, rising to 62.5% after 24 months, and remaining stable at 64.3% at 36 months and 72 % at 48 months. Out of the 27 patients total who required a repeat fulguration, 18/27 (66%) received one within 12 months and 25/27 (93%) received one within 24 months. Of the 6 patients that progressed to cystectomy, the mean number of prior fulgurations was 2.5 and the mean time between initial fulguration and cystectomy was 24.1 months. CONCLUSIONS: IC/PBS patients with Hunner lesions can be effectively treated with fulguration, however a majority of Hunner ulcer phenotype IC/BPS will require repeat fulguration within 2 years and may benefit from close followup. A small percentage will progress to cystectomy.
