We report a case of a 43-year-old man with bilateral distal biceps tendon rupture while performing pull-ups undergoing a bilateral simultaneous surgical repair with a cortical suspension device couple with an interference screw. At 5-month follow-up each elbow regained full ROM and strength. The patient returned to the preinjury level of sport and participated with excellent results in gymnastic competitions. The simultaneous reinsertion of both distal bicep tendons with this technique leaded to a very fast recovery with good clinical outcomes despite the complexity of the case and the presence of multiple risk factors.
