Due to the increased longevity of patients and advances in diagnostic modalities, an increase in the prevalence of patients with orbital metastasis is being observed. Here, we report a lady in her 40s who presented with complaints of painless protrusion of the left eyeball with restricted motility and reduced vision. She gave history of surgery for hemorrhoids. Based on clinical and imaging findings, a diagnosis of orbital metastasis was made. Thorough systemic evaluation revealed the primary tumor to be a melanoma of anorectal origin with widespread metastasis. An orbital surgeon should keep high suspicion for metastasis while working up cases of orbital malignancy, especially in the elderly population.
