Nevus sebaceous is a benign cutaneous hamartoma involving the epidermis, pilosebaceous units, and other adnexal structures. To avoid or minimize the scar, photodynamic therapy (PDT) is recommended as a non-surgical treatment. In this report, we present a successful case of recurrent nevus sebaceous treated with a combination therapy of PDT and surgical excision. The patient was followed-up for a period of 6 months to evaluate the effectiveness of the treatment. The patient underwent three sessions of PDT at three-week intervals, followed by surgical resection. Following the procedure, there was no recurrence of the lesion after 6 months, and the achieved a favorable cosmetic outcome. In cases where the nevus sebaceous affects larger areas, employing a short-term PDT regimen in conjunction with surgical intervention proves to be an effective strategy, reducing the extent of surgical excision and yielding superior cosmetic results for our patients.
