Management of elbow arthritis in younger and higher demand patients is challenging. Interposition arthroplasty of the distal humerus is a well-described procedure whose results are highly dependent on elbow stability. We propose an interposition technique that employs an Achilles allograft as both an interposition material as well as a source of simultaneous bidirectional ligament reconstruction. By resurfacing one articulating surface and re-stabilizing the elbow, longevity may be improved. This technique may be indicated for treatment of post-traumatic or primary osteoarthritis of the elbow. The step-by-step instructions for surgical implantation of this interposition arthroplasty are described and illustrated.
