Background and Importance Upadacitinib is a selective and reversible inhibitor of Janus kinases (JAK), indicated in several immune-mediated diseases. The profile of patients who use this drug is very diverse and has relevant adverse reactions (ARs) of which it is important to closely monitor. Aim and Objectives Describe the profile of patients treated with upadacitinib, its conditions of use and its safety profile in a tertiary hospital. Material and Methods Descriptive and retrospective study of all patients treated with upadacitinib between 01/02/2021 and 06/30/2023.The variables collected were: age, sex, pathology, dosage, ARs, duration of treatment, toxic habits, previous treatment and adherence. Data were extracted from the outpatient module (Farmatools®) and the computerised electronic history (Jimena®).The data collection and analysis was carried out in a Microsoft Excel table of our own creation. Results 34 patients were included (9 were excluded due to off-label use). Half (17) were men. Mean age of 41.03±13.76 years. 12 (35.29%) had toxic habits and 15 (44.11%) were in psychiatric treatment, of which 10 (66.67%) suffered from a rheumatic pathology. They were prescribed for: 2 (5.89%) ankylosing spondylitis, 5 (14.70%) inflammatory bowel disease, 9 (26.47%) atopic dermatitis, 5 (14.70%) psoriatic arthritis and 13 (38.24%) for rheumatoid arthritis (RA). 10 (29.41%) discontinued treatment due to lack of efficacy. 8 (23.53%) started with a dose of 30mg/24h. On average they had 2.23+-1.71 previous biologics. The median duration of treatment in these patients was 144 (49–435) days and 7 (20.58%) had <90% adherence. The main AEs were: 3 (8.82%) lipid alterations and 7 (20.59%) infectious process. In no patient did it have to be discontinued due to severe AR. Conclusion and Relevance The majority of upadacitinib in our hospital is prescribed for rheumatology, specifically for RA, and a significant number of these patients suffer from some psychiatric pathology. The most common dose is 15mg/24h.The safety profile is good, although the main AR found is an alteration of the lipid profile that, if the high percentage that has some toxic habit is taken into account, can negatively affect cardiovascular health.
