Background At Dudley Group NHS Foundation Trust (DGFT) we have implemented the Gold Standards Framework (GSF) to support identification of patients in the last year of life and development of an individual plan of care. Since implementation commenced in 2019, 8 adult inpatient wards have achieved GSF accreditation with Coronary Care and Critical Care both being the first speciality wards nationally. In the community the Rapid Response community team have achieved accreditation which is again a first. As we know 30% of adult in patients are in the last year of life and the metrics/outcomes collected support the further implementation of GSF and assurance regarding quality of care. Method Using PowerBI local metrics regarding GSF can be reviewed at regular intervals and reported at ward, divisional and Trust level. The data is collected from an electronic GSF document completed during admission with gathers information including identification and preferred place of care. Results The metrics have identified an increase in identification of patients that are in the last year of life and for April 2023 had increased to nearly 17% and we have agreed a target of 20% by April 2024 at Trust end of life care working group. Furthermore, in April 2023 59% of patients identified as GSF red or amber achieved preferred place of care on discharge. Conclusion Implementing the GSF across DGFT has had measurable impacts on the metrics and patient outcomes which are reported through divisions and targets agreed at Trust end of life care working group. Due to the improvements the targets have been increased. For example, achievement of preferred place of care on discharge has been increased from 60% to 70%.
