True aneurysms arising from the previous arterial anastomotic site are a rare entity. Usually seen in the distal part of a vein graft, as the arterialized vein exhibits different characteristics compared to the arterial wall histologically, making it more susceptible to aneurysm formation. These aneurysms pose a risk of rupture, bleeding, and distal ischemic features. Operative intervention is indicated for relief of symptoms, correction of the cosmetic defect, and to prevent rupture. Surgical reconstruction is the preferable choice for treating para-anastomotic aneurysms.
