The performance of health sector officials is a crucial issue as it is directly related to the quality of public services. However, data indicate a gap between the targets and the actual performance of Health Department employees, particularly in terms of responsiveness, speed, and innovation. The theoretical framework employed is the Ability-Motivation-Opportunity (AMO) model, combined with the concept of OCB as a mediating mechanism. The method applied is a Systematic Literature Review (SLR), analyzing 11 scientific publications from the 2024–2025 period. The findings reveal that organizational culture, transformational leadership, and competence are dominant factors influencing performance. Furthermore, OCB plays a significant mediating role by fostering extra-role behaviors that enhance collaboration, innovation, and the effectiveness of public service delivery in the Health Department.
