Case report shows challenges and strategies for anesthetic management in thyroid surgery with dilated cardiomyopathy, highlighting the importance of preoperative planning.
Anesthetic management of a patient with a large thyroid swelling and coexisting dilated cardiomyopathy (DCMP) presents significant perioperative challenges. The combination of airway compression and compromised cardiac function necessitates meticulous preoperative planning, intraoperative vigilance, and postoperative care. We present the case of a 52-year-old male with a large solitary thyroid nodule, euthyroid, and Bethesda II, causing tracheal deviation and compression, along with a history of DCMP, undergoing open left hemithyroidectomy. The anesthetic strategy included a balanced approach to maintaining hemodynamic stability, ensuring a patent airway, and minimizing cardiac stress.
