This retrospective real-world study analyzed Body Mass Index (BMI) and obesity in 257 Spanish children/adolescents with HIV receiving DTG-based or non-INSTI regimens over five years. DTG-based regimens increased BMI Z-scores without clear clinical relevance. Continuous monitoring of weight trajectories is essential to protect long-term cardiovascular health in this population.
