Panorama: Studies on the erectile function of the clitoris are scarce, and the information in the literature is conflicting. Aims: To gather evidence on the reality or not of clitoral erection and discuss the findings in light of current theories of sexuality. Method: Systematic review of the PubMed, SciELO, LILACS, and Wiley databases, using the terms "clitoris" and "erection" in August of this year. Results: 54 articles were returned from PubMed, 28 from SciELO, 124 from LILACS, and 444 from Wiley. 200 were included, leaving 8 after exclusions. Conclusion: The clitoris is an erectile organ with effective erection. It has functional corpora cavernosa and spongiosum, with arterial and venous complexes and striated muscles responsible for blocking this blood flow, causing erection. This erection is alienated from knowledge, including scientific negligence, which may be masking a pandemic of female erectile dysfunction. Correcting the current state of the art, both scientifically and in popular understanding, is urgent.
