Abstract The inflammatory myofibroblastic tumor (IMT) is a soft-tissue lesion composed of myofibroblastic proliferation accompanied by mixed inflammation. These tumors have intermediate aggressive potential and may have local recurrence. A comparison of histomorphological features of benign lesions like IMTs mimicking malignancy in 2 pediatric soft-tissue lesions was sent to the department. An immunohistochemistry (IHC) panel and molecular workup were performed for both cases as required. A significant histomorphological overlap between benign pediatric diseases such as tuberculosis, Kikuchi’s Kimura disease, and IMTs is seen. The above case is interesting as the IHC came out to be negative on IHC for anaplastic lymphoma kinase, but molecular studies helped diagnose the case and lead to its timely management of the case.
