Aneurysmal dilatation of the sinus of Valsalva is a rare clinical condition with diverse and serious complications. We report a unique case of a left sinus of Valsalva aneurysm (SOVA) causing coronary compression and ischemia, followed by an embolic stroke. A 73-year-old man presented with exertional chest pain. Computed tomography angiography revealed a large left SOVA compressing the coronary arteries. During hospitalization, the patient experienced recurrent embolic events and ultimately died before aneurysm repair. This case highlights the diagnostic challenge of SOVA and underscores the importance of early imaging and timely surgical intervention.
