Abstract Failed Back Surgery Syndrome (FBSS) refers to persistent or recurrent pain following spinal surgery. Managing FBSS is often difficult with conventional treatments. To evaluate the efficacy of ozone nucleolysis in patients diagnosed with FBSS. This case series included six patients with FBSS treated with ozone nucleolysis at the Department of PMR, RIMS, Imphal, from October 2022 to July 2024. Patients were assessed using the Visual Analog Scale (VAS) and Oswestry Disability Index (ODI) at baseline and after 1, 3, and 6 months. All six patients demonstrated marked improvement in pain and functional status post-treatment. MRI findings showed favorable changes in disc structure in some cases. Ozone nucleolysis appears to be a safe and effective intervention for FBSS, offering significant symptom relief and functional recovery.
