This article critiques rigid evidence hierarchies, highlighting limitations of randomized controlled trials, systematic reviews and meta-analyses undervaluation of qualitative research. It calls for contextualizing study designs, addressing publication bias, adopting the Grading of Recommendations, Assessment, Development, and Evaluations framework, and centering patient values to evolve evidence-based medicine into a more inclusive, patient-centered practice.
