A 27-year-old woman presented with a progressively enlarging subcutaneous swelling on the arm, with prior periocular involvement, and imaging revealed a hypoechoic lesion containing characteristic bilaminar echogenic structures. Surgical excision yielded a 13 cm Dirofilaria repens worm, with no microfilariae detected in blood, and antiparasitic therapy resulted in complete resolution. This case underscores the importance of early clinical suspicion, supportive imaging, and timely surgical removal in managing human dirofilariasis.
