This case report describes the diagnosis and treatment of migrating subconjunctival Dirofilaria in a 61-year-old male patient who presented with symptoms of left eye redness and a sensation of a moving foreign body. The larva was surgically removed and identified as Dirofilaria repens through histopathology. The patient recovered well and was referred for systemic examination. Ocular dirofilariasis is an emerging zoonotic infection, though rare, and is particularly relevant for ophthalmologists from endemic areas, as they should be aware of this disease to increase recognition and reporting of dirofilariasis. Additionally, proper identification of specific species of Dirofilaria is crucial for appropriate management.
