Abstract Pembrolizumab is an immune checkpoint inhibitor that blocks the PD-1 pathway on T-cells. It has been associated with a variety of immune-related adverse events, including Guillain–Barré syndrome (GBS), a rare but critical event. We report the case of a patient with early-stage metaplastic breast cancer who developed Grade 4 pembrolizumab-induced GBS early in the treatment course and had a remarkable response to intravenous immunoglobulins and steroids.
