Selvester score ≥3 predicts LGE with ~85% specificity in HCM.
Why the study?
The Selvester score may assist in diagnosis and treatment planning alongside cardiac MRI, but its ability to predict myocardial scar identified on cardiac MRI in patients with HCM needed evaluation.

The Selvester score derived from standard ECGs may serve as a cost-effective tool to predict myocardial scarring in patients with hypertrophic cardiomyopathy.
May support ECG scar screening in hypertrophic cardiomyopathy; hypothesis-generating and should not yet change practice.
Introduction The Selvester score is an electrocardiographic system designed to estimate the extent of myocardial scarring caused by ischaemic or non-ischaemic conditions. When used in conjunction with cardiac magnetic resonance imaging (MRI), it may assist in diagnosis and treatment planning. This study aimed to examine the relationship between the Selvester score and the presence of myocardial scarring identified on cardiac MRI in patients with hypertrophic cardiomyopathy (HCM). Purpose To evaluate whether the Selvester score can predict myocardial scar detected by cardiac MRI in patients with HCM. Methods This retrospective study included 36 patients with HCM who underwent cardiac MRI within the past year. Electrocardiograms (ECGs), MRI findings, and laboratory parameters were reviewed. Selvester scores were calculated and compared with the presence of late gadolinium enhancement (LGE) on MRI, which served as an indicator of myocardial scar. Results Of the 36 patients, 28 (77.7%) demonstrated LGE on cardiac MRI. No significant differences were found in baseline clinical features between patients with and without LGE. However, pro-BNP levels were significantly higher in the LGE-positive group (p = 0.003). Selvester scores were also significantly elevated in patients with LGE (p = 0.031). A Selvester score ≥3 predicted LGE with a sensitivity of 57% and specificity of 85% (AUC: 0.69, 95% CI, p = 0.03). Conclusions The Selvester score may serve as a useful, cost-effective tool for estimating myocardial scarring in patients with HCM. These findings support its potential role in predicting myocardial fibrosis as confirmed by cardiac MRI, and its application in clinical risk stratification.

Serifler et al. (2026) conducted an observational in hypertrophic cardiomyopathy (HCM) (n=36). Selvester score was evaluated on Presence of late gadolinium enhancement (LGE) on MRI (AUC 0.69, p=0.03). A Selvester score ≥3 predicted late gadolinium enhancement on cardiac MRI in patients with hypertrophic cardiomyopathy with a sensitivity of 57% and specificity of 85% (AUC 0.69, p=0.03).
