TEER reduces mitral regurgitation in a cardiac amyloidosis case series, though early mortality remains high.
Why the study?
Cardiac amyloidosis can cause complications such as mitral regurgitation, but the efficacy of transcatheter mitral edge-to-edge repair in patients with amyloidosis remains uncertain.
Does transcatheter mitral edge-to-edge repair reduce mitral regurgitation and improve outcomes in patients with transthyretin cardiac amyloidosis?

Does transcatheter mitral edge-to-edge repair reduce mitral regurgitation and improve outcomes in patients with transthyretin cardiac amyloidosis?
While TEER can acutely reduce mitral regurgitation in patients with transthyretin cardiac amyloidosis, long-term outcomes remain poor due to disease progression and comorbidities, highlighting the need for careful patient selection.
Acute MR reduction with TEER in TTR amyloidosis does not ensure survival; leaves open whether careful selection improves outcomes in larger studies.
Cardiac amyloidosis is characterized by the deposition of amyloid fibrils, which cause complications such as mitral regurgitation (MR). The efficacy of transcatheter mitral edge-to-edge mitral repair (TEER) in patients with amyloidosis remains uncertain. We herein present four cases of TEER for transthyretin cardiac amyloidosis. TEER reduced the MR in all cases. However, two of the four individuals died within one year, and in one case, a recurrent MR deterioration occurred. Although TEER can reduce MR in individuals with cardiac amyloidosis, the long-term outcomes appear to be poor owing to disease progression or comorbidities, thereby underscoring the need for careful patient selection.

Ujita et al. (2026) conducted a case report in transthyretin cardiac amyloidosis (n=4). transcatheter mitral edge-to-edge repair was evaluated on mitral regurgitation reduction. Transcatheter mitral edge-to-edge repair reduced mitral regurgitation in all four patients with transthyretin cardiac amyloidosis, though two patients died within one year after the procedure.
