Case report reveals delayed unilateral posterior polar cataract after lightning strike, suggesting unique lens damage.
Lightning-induced cataracts are uncommon and usually present as anterior or posterior subcapsular opacities. We report a rare case of delayed-onset unilateral posterior polar cataract (PPC) following an indirect lightning strike, not previously described in the literature. A 41-year-old male developed gradual visual decline in his left eye over 12 years, with examination revealing PPC, surrounding posterior subcapsular cataract, and iris atrophy. The posterior segment was normal. Manual small incision cataract surgery was performed successfully, restoring 6/6 vision. This case highlights the potential for delayed localized lens damage after lightning and the role of meticulous surgery in achieving full visual recovery.
