Ophthalmia nodosa (ON) is an ocular inflammatory reaction to insect hairs. Type 5 ON involves vitreoretinal structures and usually has a fulminant course requiring surgery. We report a case of 23-year-old male with an inferotemporal retinal lesion with peg-like extensions; optical coherence tomography showed hyperreflective vitreous opacities and cystoid macular edema (CME). History revealed insect contact 6 years earlier. CME resolved within 4 weeks after posterior sub-tenon triamcinolone, with vision improving from 6/12 to 6/6. This case highlights the value of multimodal imaging and detailed history in diagnosing ON and demonstrates that conservative management can achieve favorable outcomes in select cases.
