Abstract Background The gender-related difference on clinical outcomes in patients with HeartMate 3 LVAD in Japanese cohorts remains unclear. Purpose This study examined the clinical impact of gender difference on clinical outcomes among patients with HeartMate 3 LVAD using the prospective multi-center J-MACS registry data. Methods We included patients with advanced heart failure who had received HeartMate 3 LVAD implantation and were registered for the J-MACS registry between 2019 and 2023. Cumulative mortality and adverse events leading to rehospitalization were retrospectively compared between genders. Results A total of 465 patients (mean 50 y; women: 26.5%) were included. There was no significant difference in cumulative mortality between genders during a median follow-up of 964 days (p = 0.499). However, women exhibited a higher incidence of rehospitalization due to major bleeding events, with an adjusted hazard ratio of 3.224 (95% confidence interval: 1.450–6.940, p = 0.028, figure). Conclusions Gender difference demonstrated an increased risk of major bleeding requiring hospitalization, whereas did not affect overall mortality following HeartMate 3 implantation. Further studies are warranted to elucidate the mechanisms underlying the increased rehospitalization risk for major bleeding in women.
