Abstract Background South Korea has experienced a significant decline in coronary artery disease mortality over the past 30 years, positioning it among countries with the lowest mortality rates. However, mortality statistics alone do not fully capture the actual disease burden. Previous efforts to estimate acute myocardial infarction (AMI) incidence using National Health Insurance (NHI) claims data faced challenges in differentiating new occurrences from existing cases based solely on diagnostic codes. To overcome this limitation, we developed an algorithm to accurately identify first and recurrent AMI events within NHI claims data, enabling effective monitoring of national trends. Methods Our algorithm integrates admission and discharge dates with claim codes for primary and secondary diagnoses, diagnostic tests, procedures, and surgeries to systematically identify first and recurrent AMI events within the NHI Big-Data platform, which encompasses the entire population. Validation was conducted through a retrospective review of 1,399 medical records from 24 hospitals nationwide, yielding a positive predictive value (PPV) of 92% for first AMI events and 78% for recurrent AMI events. Incidence rates are presented as crude and age-standardized rates (reference: 2005 Korean population) per 100,000 person-years. Results The total number of AMI events increased from 22,395 (20,923 first and 1,472 recurrent cases) in 2011 to 34,969 (31,604 first and 3,365 recurrent cases) in 2022. The crude incidence rate of total AMI events rose from 46.8 to 67.9 per 100,000 person-years, while the age-standardized incidence rate increased from 44.7 to 58.5 per 100,000 person-years. Incidence rates climbed steadily in both men and women until 2019, followed by a temporary decline in 2020, likely due to health behavior change and healthcare disruptions during the COVID-19 pandemic. In men, incidence continued to rise through 2022, whereas in women, AMI incidence declined after 2019. The male-to-female ratio in total AMI incidence escalated from 2.1 in 2011 to 2.9 in 2022. Conclusion Over the past decade, AMI incidence in South Korea has shown a consistent upward trend, with a brief decline in 2020, potentially associated with the COVID-19 pandemic's impact.
