Export Breast carcinoma metastasizing to the gallbladder is an extremely rare occurrence. Here, we present the case of a 57-year-old lady who was diagnosed with invasive lobular carcinoma of the breast. She underwent breast conservative surgery, followed by adjuvant chemotherapy, radiation therapy, and hormonal therapy. Eighteen years later, she developed a local recurrence, and a workup revealed an incidental thickened gallbladder wall associated with soft tissue swelling. She underwent a mastectomy and laparoscopic cholecystectomy. Histopathological examination revealed metastasis from lobular breast cancer to the gallbladder. Being considered a patient with metastatic breast carcinoma, she was subjected to letrozole and ribociclib-based palliative endocrine therapy.
