Abstract Primary leiomyosarcoma of the scalp is rare in clinical practice. Being an aggressive tumor, a prompt diagnosis is important for the definitive management. Hereby, we present a rare case of a 37-year-old female who presented with progressive swelling over the scalp and was later diagnosed with leiomyosarcoma, grade 3, on final histopathology after the surgical excision. The patient is on regular follow-up after completion of adjuvant radiotherapy and at 6 months follow-up she is asymptomatic without any evidence of recurrence of the disease clinico-radiologically. The present report highlights the leiomyosarcoma as a differential diagnosis for scalp masses, and early diagnosis may prevent locally invasive growth as well as distant metastasis.
