Background: Recurrent pregnancy loss (RPL) can have multifactorial causes, including anatomical, genetic, and energetic imbalances. The Du (Governing) channel, associated with the spine and nervous system, may play a role in fertility through its influence on Qi flow, nerve conduction, and organ alignment. Scar tissue from spinal surgeries and structural misalignments could potentially disrupt these functions. Case: A 31-year-old woman with a history of congenital scoliosis and two early miscarriages (6–8 weeks) presented for treatment. Despite normal genetic testing, both pregnancies ended due to abnormal fetal heartbeats. She had spinal fusion surgery (T8–L2) at age 19, which resulted in an extensive scar tissue (15 cm long, 0.5–1 cm wide) that causes tightness, numbness, and weather-sensitive pain. Otherwise, she was healthy, with normal body mass index, digestion, and occasional headaches. Treatment consisted of (1) acupuncture treatment based on four phases of the menstrual cycle, (2) targeted scar tissue therapy on the Du channel using the threading technique to improve Qi flow, nerve conduction, and fascial balance, and (3) Lin-style anterior/posterior axis alignment to address spinal structural deviations. Key acupuncture points included Du20 (Baihui), LI4 (Hegu), LV3 (Taichong), ST36 (Zusanli), SP6 (Sanyinjiao), KD3 (Taixi), Ren12 (Zhongwan), Ren6 (Qihai), Ren4 (Guanyuan), Ren3 (Zhongji), Zigong Xue, Ear Liver, and Ear Shenmen. Results: After four weekly sessions, the patient conceived naturally. A modified Shou Tai Pill formula and a 40 mg daily dose of liposomal progesterone skin cream derived from wild yam (Brand: Now Solutions) were prescribed to prevent miscarriage, along with monthly acupuncture treatments using pregnancy-appropriate points to support post-conception care. The patient gave birth to a healthy baby boy and reported an uncomplicated pregnancy, reduced headaches, and less scar tissue-related tightness and pain. Conclusion: This case highlighted the role of Du channel treatment in fertility care, suggesting that spinal scar tissue and structure misalignment may obstruct Qi flow, impair nerve conduction, and alter organ positioning, contributing to RPL. Further research is needed to explore Du channel’s role, scar tissue therapy, and structural alignment as adjuncts to fertility treatment.
