Key Points

  • To determine if spironolactone, in combination with an ACE inhibitor, can prevent left ventricular remodeling after myocardial infarction.
  • Participants received either spironolactone with ACE inhibitor or ACE inhibitor alone.

Structured PICO

Does immediate administration of spironolactone combined with an ACE inhibitor prevent post-infarct left ventricular remodeling in patients with a first anterior acute myocardial infarction?

P
Population
Patients with first anterior acute myocardial infarction
I
Intervention
Immediate administration of mineralocorticoid receptor antagonist (spironolactone) combined with an ACE inhibitor
C
Comparator
ACE inhibitor alone
O
Outcome
Post-infarct left ventricular remodeling and suppression of a marker of myocardial collagen synthesissurrogate

Immediate administration of spironolactone on top of ACE inhibitors helps prevent adverse left ventricular remodeling following a first anterior acute myocardial infarction.

Abstract

These findings indicate that MRA combined with ACE inhibitor can prevent post-infarct LV remodeling better than ACE inhibitor alone in association with the suppression of a marker of collagen synthesis.
