Abstract Coronary computed tomography angiography (CCTA) has evolved into a cornerstone modality for the non-invasive assessment of coronary artery disease (CAD). Advances in detector technology, image reconstruction, and functional post-processing have enabled high-fidelity visualization of coronary anatomy, plaque morphology, and flow dynamics at radiation doses well below 1 mSv. CCTA now plays a pivotal role not only in the diagnosis of CAD but also in prognostic risk stratification and management guidance. Techniques such as CT-derived fractional flow reserve (FFRCT), quantitative plaque analysis, and artificial intelligence (AI) -driven image interpretation are expanding its role toward comprehensive anatomical and functional evaluation. This review summarizes the technical developments, clinical applications, prognostic value, and emerging innovations of CCTA, highlighting its integration in modern patient management and its potential within precision cardiovascular medicine.
