This study focuses on evaluating the cost-effectiveness of district hospitals in Tanzania by applying multilevel regression analysis. Multilevel regression analysis was employed to evaluate cost-effectiveness metrics across different levels of health care delivery in Tanzania. The study utilised data from district hospitals over a two-year period (-). The multilevel regression analysis revealed that the proportion of patients receiving appropriate treatment varied significantly between districts, with some areas showing substantial improvement. This research contributes to the understanding of healthcare system performance in resource-constrained settings by providing a robust method for assessing cost-effectiveness at multiple levels within district hospitals. The findings suggest that targeted interventions and increased funding could enhance treatment outcomes, thereby improving overall service delivery efficiency. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
