Introduction: Historically, HIV patients were often excluded from implant therapy due to immune and healing concerns. Widespread ART use has improved candidacy. Methods: Narrative review synthesizing clinical evidence on implant safety, success, and prosthodontic considerations in HIV-positive individuals. Results: When clinically stable on ART, HIV-positive patients show implant outcomes comparable to immunocompetent populations. Important factors include CD4+ counts, viral load control, comorbidities, and adherence. Conclusions: With appropriate assessment and individualized care, implant therapy is a viable option for many HIV-positive patients. Emphasis on stigma-free, evidence-based, patient-centred prosthodontic protocols is warranted.
